Terms and Conditions – Farnham Weight Loss Clinic

These Terms and Conditions (“Terms”) govern the services provided by Farnham Weight Loss Clinic (“the Clinic”, “we”, “our”, “us”). By booking or receiving treatment, you agree to be bound by these Terms.
1. About Us
Farnham Weight Loss Clinic is a GP-led private medical service offering weight management consultations, treatments, and ongoing support, including the direct supply of GLP-1 medications. Farnham Weight Loss Clinic is trading name for Ishmail Boakye-Acheampong limited which is a CQC registered company

2. Eligibility for Treatment
- Services are available to individuals aged 18 years and over.
- Treatment is subject to a full medical assessment and the clinician’s professional judgement.
- We reserve the right to refuse or discontinue treatment if it is not clinically appropriate, safe, or in line with current medical guidelines.
3. Medical Disclaimer
- Our treatments support weight management but do not guarantee results.
- Outcomes depend on factors such as lifestyle, diet, and adherence to treatment.
- Our services do not replace general NHS or GP care — patients must continue routine health checks with their usual GP.
- Patients must disclose full and accurate medical information, including current medications, allergies, and health conditions.
4. Consultations & Supply of Medications
- All new patients must attend an initial consultation (see Section 5: Payment).
- If clinically appropriate, GLP-1 medications will be supplied directly by the Clinic.
- External prescription may be issued or patients may receive their medication supply from us as part of their treatment plan but will be charged separately from the consultation fee.
- Medication supply is subject to stock availability and may vary depending on type and dosage.
- Patients must use medications only as directed and must not share them with others.
5. Packages & Payment
- Initial Consultation Fee:
- The initial consultation fee is £250, payable in advance.
- This includes a medical assessment and discussion of weight management options.
- The cost of GLP-1 medication is not included in this fee.
- Treatment Packages:
- After the initial consultation, suitable packages may be offered:
- Gold Package – 3 months
- Platinum Package – 6 months
- Diamond Package – 12 months
- Packages include scheduled follow-up consultations, clinical monitoring, and direct supply of medication (charged separately).
- Medication charges: GLP-1 medication is billed separately based on type and dose supplied.
Payment Terms:
- Payments must be made in advance (upfront or monthly instalments, depending on agreement).
- Appointments or medication supply will not proceed without payment.
- Prices are reviewed periodically; patients will be notified of any changes.
6. Cancellations & Refunds
- Appointments cancelled with less than 48 hours’ notice may be charged in full.
- The £250 consultation fee is non-refundable once the consultation has been delivered.
- Packages are non-refundable once treatment and medication supply have commenced, except at the Clinic’s discretion.
- If treatment is stopped for clinical safety reasons, a pro-rata refund may be considered for unused services.
7. Patient Responsibilities
Patients agree to:
- Follow treatment instructions and lifestyle advice.
- Attend all scheduled follow-up and monitoring appointments.
- Store medication safely and securely, as advised.
- Report any side effects or health changes promptly.
- Not share or redistribute medication under any circumstances.
8. Clinic Responsibilities
The Clinic will:
- Provide safe, evidence-based care in line with GMC and CQC standards.
- Supply licensed medications from approved sources.
- Ensure confidentiality and protection of patient data.
- Clearly explain risks, benefits, and alternatives before treatment.
- Provide aftercare support and respond promptly to concerns.
9. Liability
- The Clinic is not responsible for weight loss outcomes where treatment plans are not followed.
- We accept no liability for harm caused by withheld medical information, undisclosed conditions, or misuse of supplied medication.
- Nothing in these Terms excludes liability for death or personal injury caused by our negligence.
10. Complaints
Complaints should be submitted by email to: admin@farnhamweightloss.com.
- We aim to acknowledge complaints within 3 working days and investigate them promptly in line with our Complaints Policy.
11. Governing Law
These Terms are governed by the laws of England and Wales. Any disputes are subject to the exclusive jurisdiction of the English courts.
